Fundraiser/Activity Request Forms

Saint Joseph Catholic Church is committed to supporting ministry events while ensuring good stewardship, transparency, and coordination across the parish.

Use the forms below to request approval for ministry activities and fundraisers, review parish fundraising policies, submit annual ministry plans, and complete required post-event reporting. All ministries are required to follow the parish fundraising and activities guidelines approved by the Parish Finance Council. Fundraiser requests must be submitted by June 1 for the upcoming fiscal year, while activity requests may be submitted by June 1 or at least 30 days before the proposed event date.
